If you are new to the area please come into the surgery to collect registration forms. We will make an appointment for you to have a new patient review.
If you are going to be with us for less than 3 months then we can treat you as a temporary resident but you will stay registered with your usual GP.
You will also need to provide copies of the following documentation to prove your eligibility to receive NHS treatment.
- Proof showing normal country of residency (i.e. Passport, EHIC card)
- Proof of address (i.e. utility bill within the last 3 months (not mobile phone), tenancy agreement)
- Non EEA applicants also require one of the following:
- Work Visa**
- Student Visa & Proof of attending college/university**
** Spouse of one of the above will require proof of marriage / civil partnership
** Dependants will require their birth certificates (under 16 or 19 if in full time education – proof required)
